When thinking of a design concept, try to make it fun and get everyone involved in it. A great way to do that is to have a family party and then have a design concept contest. Everyone who wants to do it can present a design idea, and you can all vote on a winner.
Then, when you have the design idea, you can take it directly to the printing company and they can tweak it a little so that it fits perfectly on the shirt. Once you approve the design, they can print it and you should be good to go.
